Patients enrolled in Medicaid have worse survival rates than those with private insurance or even no insurance at all, according to a new study focused on Ohio Medicaid recipients published in the journal Cancer.
Researchers Siran Koroukian, Paul Bakaki, and Derek Raghavan compared survival and five-year mortality with Medicaid status in more than 11,000 Ohio adults aged 15 to 54 years and diagnosed in the years 1996-2002 with eight highly treatable cancers. They also sorted the Medicaid enrollees into two categories—those enrolled at the time of their diagnosis and those enrolled afterwards—to get more insight into how coverage impacts outcomes. . . . . . .

Obamacare includes such a variety and volume of negative policies that it’s hard to keep track of them all. Here is a list of 10 terrible provisions that every American should be aware of:
1. It increases taxes on families earning over $250,000. In 2013, the employee portion of the Medicare payroll tax will increase from 1.45 percent to 2.35 percent for families earning $250,000 or more and individuals earning $200,000 or more. The income threshold is not indexed for inflation, so more and more middle-income families will be hit by the tax hike as time goes on.

- It adds a new tax to investment income. The increased payroll tax rate is also applied to high-earners’ investment income for the first time beginning in 2013. It will hit capital gains, dividends, rents, and royalties, discouraging investment and harming economic growth.
- It puts new limitations on those with HSAs and FSAs. Starting in 2012, Obamacare restricts the products that consumers may purchase with a Health Savings Account (HSA) or Flexible Savings Account (FSA)—such as over-the-counter medications—and increases the penalty for such non-qualified uses of HSAs. It also limits the amount taxpayers may deposit into an FSA to $2,500 a year in 2013.
- It adds a new tax on those who purchase medical devices. In 2013, a 2.3 percent excise tax will be applied to medical devices, causing a $28.5 billion tax hike on medical device manufacturers. The industry will pay for this tax by reducing jobs and passing additional costs on to consumers.
- It penalizes marriage. Obamacare creates new taxpayer-funded subsidies for the low and middle classes to purchase health coverage, but the structure of the subsidies allows two individuals to claim more in subsidies alone than if married. This discriminates against married couples and discourages marriage at almost all age and income levels.
- It violates religious liberty. The Department of Health and Human Services included the full range of contraceptives, including abortion-inducing drugs, among the women-specific preventive services that Obamacare requires insurers to include with no cost-sharing. This mandate violates Americans’ conscience rights and religious liberty. Its narrow exemption for religious employers will force many who find these products morally objectionable—including religious charities, hospitals, and schools—to pay for them.
- It puts Medicare decisions in the hands of an unelected board. The Independent Payment Advisory Board, a board of 15 unelected officials, will have the power to cut Medicare spending without congressional approval. These unaccountable government appointees will be able to restrict seniors’ access to providers, treatments, and services.
- It puts a premium tax on health insurers. Obamacare adds a premium tax on health insurers that offer full coverage beginning in 2014. On average, the tax is expected to increase premiums by 1.9 percent to 2.3 percent in 2014 and between 2.8 percent and 3.7 percent by 2023. Combined with the other provisions in Obamacare, this tax will have a huge impact on the cost of premiums.
- It creates a new unsustainable entitlement program. On top of Social Security, Medicare, and Medicaid, Obamacare created a new long-term care entitlement called the CLASS program. It is actuarially unsound, unworkable, and unsustainable. As a result, the Administration has already put its implementation “on hold.”
- It puts over half of all Americans on a government program. Because of Obamacare’s huge expansion of Medicaid and creation of taxpayer-funded subsidies to purchase health coverage, more than half of all Americans will be dependent on a government health care program (Medicare, Medicaid, or the government exchanges) by the end of this decade.

If Obamacare is completely implemented, doctors will no longer be practicing medicine. They will instead become the drones tasked with deciding who gets the meager healthcare crumbs doled out by the bureaucrats who have the ultimate power over patient life and death. Those who are deemed to have illnesses that require treatments which are not cost effective can expect a one way ticket to a hospice.
Like so many bills passed by Congress, there was a hidden provision in the Stimulus bill passed in 2009. It spends 1.1 billion dollars to create an important piece of the framework for the healthcare bill called the Coordinating Council on Comparative Effectiveness Research. It is based on the false premise that doctors in consultation with their patients don’t have the ability to make the right healthcare choices (see executive summary). The council consists of 15 people appointed by the President.
They all have one thing in common–they are all isolated from day to day patient care; and therefore, are insulated from the real practice of the art of medicine. It makes it easy to see patients as a cost center to be controlled. With views of members like Dr Emanuel, who champions the complete-lives system, it is hard to ignore the probability that senior citizens, those with chronic illness, and the very young will be on the outside looking in. This council is another example of the people of this country being told by the government that it knows what is best for us. . . . .

Peter Orszag, formerly the Director of the White Office of Management and Budget, appears to have found a new career as a pundit for the NYT. The fact that he's chosen to admonish doctors in a recent sermon at least means that the new preacher and his chosen pulpit are well-matched. The contempt for the medical profession shown by Mr. Orszag -- and his boss -- during health care reform is readily apparent, as he begins his editorial by stating that
[d]octors, like most people, don't love to work weekends, and they probably don't enjoy being evaluated against their peers. But their industry can no longer afford to protect them from the inevitable. Imagine a drugstore open only five days a week, or a television network that didn't measure its ratings. Improving the quality of health care and reducing its cost will require that doctors make many changes - but working weekends and consenting to quality management are two clear ones. . . . .

A couple of weeks ago the President and his allies in the press made much of ObamaCare's six-month anniversary, using the occasion to tout a selective list of minor provisions that went into effect on September 23. We heard a lot about dastardly insurance industry practices, such as the fiendish refusal to insure people against maladies they have already contracted, from which we are now shielded by the Patient Protection and Affordable Care Act (PPACA). There was no mention, however, of a more important provision that has also gone into effect. This obscure provision of PPACA, buried deep in Section 6402, has received little attention from health policy wonks and even less from the "news" media. Nonetheless, it has very serious implications for the legal rights of health care providers, the American justice system and ability of many patients to access medical care.
A fundamental principle of our justice system holds that a person cannot be convicted of a felony unless it has been proven that he acted with "criminal intent." It must be shown, in other words, that he intentionally engaged in conduct he knew to be illegal. This is a venerable legal principle whose pedigree reaches back to the beginnings of Western Civilization, and it was an important element of the English common law upon which the American justice system is based. The requirement to prove criminal intent has, however, been something of a nuisance to Beltway bureaucrats seeking to bend private industry to their will. And it has been particularly inconvenient for federal prosecutors trying to throw doctors and hospital executives in jail for violating hopelessly obscure and Byzantine regulatory statutes. But that pebble has been removed from the federal shoe by ObamaCare. . . . .

Trade groups representing restaurants and retailers say low-wage employers might halt their coverage if the government doesn't loosen a requirement for "mini-med" plans, which offer limited benefits to some 1.4 million Americans.

The group's Center for Workforce Studies released new estimates that showed shortages would be 50 percent worse in 2015 than forecast.
"While previous projections showed a baseline shortage of 39,600 doctors in 2015, current estimates bring that number closer to 63,000, with a worsening of shortages through 2025," the group said in a statement. . . .

If you get insurance through your boss: Many people who are insured through work won’t notice immediate changes to their health plans until their health plans renew, which is tied to companies’ open enrollment periods. Health plans offered through large employers usually get renewed on January 1.
But the mandates could kick in sooner for health plans sold to new entities or individuals after September 23.

Massachusetts Health Reform Fails the Disadvantaged, Study Finds

A new study looking at the effects of the 2006 Massachusetts Health Reform on access to care, health status and ethnic and socioeconomic disparities in healthcare, shows that the legislation has led to improvements in insurance coverage as well as a decline in financial barriers to care. However, to date, it has not increased people's access to a personal physician or improved their self-rated health. Neither has it reduced healthcare inequalities between ethnic or income groups.
The research by Jane Zhu from Harvard Medical School and team suggests that health reform is necessary but not sufficient to achieve equity in healthcare. The study is published online in the Journal of General Internal Medicine, published by Springer.
In 2006, Massachusetts passed comprehensive health reform legislation to expand health coverage to all its residents and has achieved near-universal coverage. However, less is known about the effects of the reform on its residents' access to care or health status, or how this legislation has impacted ethnic and socioeconomic disparities. . . . .

You all remember Donald Berwick, right? He’s the radical Obama tapped to run the Center for Medicare & Medicaid Services, despite Berwick’s past statements romanticizing Britain’s government-run health care and a 2009 declaration that we must “ration with our eyes open.”
There was no way in hell Obama could’ve got Berwick confirmed through the normal Senate channels, so he proceeded to make a “recess” appointment and shield Berwick from ever having to defend his love-affair with socialized medicine. To put this in perspective, leading the Center for Medicare & Medicaid is not some flimsy position. As the New York Times put it, Berwick now commands the healthcare spending for one-in-three Americans and “has a budget bigger than the Pentagon’s.”. . . . .
